Wednesday Night Live!

Friendship Baptist Church is excited to kickoff this fun Wednesday night program for the 2023-2024 school year. Wednesday Night Live! was planned specifically with young people in mind. Each week we'll have yummy snacks, exciting games, crazy theme nights, uplifting worship, and a message from the Bible about what kids and teens really face. Oh, and did we mention that when you come for the first time you get a chance to win $50?! We have everything planned and ready, all we're missing is YOU! If you would like a ride there is a form at the bottom of this page for a parent or guardian to fill out if you would like us to pick you up on our van route. We would love if you made plans to come join us and Wednesday Night Live! and be encouraged in the middle of your week. See you there!

I do hereby solemnly swear that I am the legal parent/guardian of the minor child and authorize him/her to participate in the following event:

Wednesday Night Live - Wednesdays 7-8pm

I grant my authorization and consent for any adult member of Friendship Baptist Church to administer general first aid treatment for any minor injuries or illnesses experienced by the minor. If the injury or illness is life threatening or in need of emergency treatment, I authorize the Supervising Adult to summon any and all professional emergency personnel to attend, transport, and treat the participant and to issue consent for any X-ray, anesthetic, blood transfusion, medication, or other medical diagnosis, treatment, or hospital care deemed advisable by, and to be rendered under the general supervision of, any licensed physician, surgeon, dentist, hospital, or other medical professional or institution duly licensed to practice in the state in which such treatment is to occur.
It is understood that this authorization is given in advance of any such medical treatment, but is given to provide authority and power on the part of the Supervising Adult in the exercise of his or her best judgment upon the advice of any such medical or emergency personnel. It is further understood that the parent/guardian will be responsible for all medical expenses involved and will not hold the Friendship Baptist Church or any member liable or responsible for the accident should one occur at anytime during the sponsored event.
